The Licensed Masters Social Worker (LMSW) will serve as a vital member of the interdisciplinary team, providing direct social work services to participants of the PACE program. The position focuses on supporting elderly and diverse populations through comprehensive psychosocial assessments, discharge planning, care coordination, and education on available community resources.
This is a day-shift role with a moderate amount of field work in South King County. Mileage and travel time between work sites are paid. The position offers flexibility in scheduling home visits and is open to both local and traveling candidates, including first-time travelers .
Conduct psychosocial assessments and develop individualized plans of care in collaboration with the interdisciplinary team.
Provide one-on-one and group counseling to participants to address social, emotional, and behavioral issues.
Coordinate services and ensure continuity of care across care settings (PACE Center, hospital, home, or long-term care).
Educate patients and caregivers regarding discharge plans, advance directives, community programs, and available resources.
Document all assessments, interventions, and care coordination in compliance with coverage criteria and regulatory standards.
Participate in case conferences and interdisciplinary team meetings.
Conduct occasional home visits within South King County to assess living situations and provide support.
Access and coordinate community, financial, and protective services (Medicaid, Medicare, Guardianship, Power of Attorney, etc.).
Experience: Minimum 1 year of experience with elderly and diverse populations (2 years preferred).
Travel: No travel assignment experience required; first-time travelers welcome.
Mileage Reimbursement: Paid for travel between work locations; commute time not reimbursed.
Autonomy: Candidate must be comfortable managing schedule for home visits.
Shift: 5x8 Day (8:00 AM 5:00 PM), Monday Friday.
On-Call: No.
Weekends: Not required.
Education:
Master's Degree in Social Work (MSW) from an accredited institution.
Experience:
Minimum 1 2 years working with geriatric or elderly populations.
Experience in case management, discharge planning, or care coordination preferred.
Prior work within long-term care, nursing home, hospice, or community health settings highly desired.
License / Certification:
Required (upon submittal): One of the following -
WA LICSW (Licensed Independent Clinical Social Worker)
WA LSWAIC (Licensed Social Work Associate Independent Clinical)
WA LASW (Licensed Advanced Social Worker)
Other: BLS (Basic Life Support) required.
